When you arrive at The Injury Clinic, you will fill out a set of new patient forms. You can make your check-in process even easier by downloading, printing and filling out these forms before you arrive. Bring them with you.
If you don’t have access to a printer, don’t worry – just fill them out when you arrive at The Injury Clinic urgent care.
